2. The Renovation Recordkeeping Checklist - This form is used to document that all workers on the job site have been trained in the proper methods of doing a renovation in a lead hazard enviroment and that all tasks have been performed to stay in compliance with the EPA's RRP Program.
3. The Test Kit Documentation Form - This form is used to record the results of all lead tests have been done in a dwelling.
4. The Renovation Notice For Common Areas - This form is used to inform occupants of a multi family dwelling when you are proposing to do renovation work on a dwelling in common areas where lead has been found.  This form is used to inform the tenants of their rights to receive the Protect Your Family From Lead In Your Home brochure before you start the renovation.
5. The Record of Tenant Notification Form - This form is to record the results of delivery of the Protect Your Family From Lead In Your Home brochure for Multi Family dwellings.  

These forms** are needed for compliance of the Eniviromental Protection Agencys RRP Program.  We have created this form in an easy 8 1/2" x 11" format that can be opened and saved in PDF* formats.  Once opened you must save before printing or altering the form in order to keep your form for future use.
